By filling out this form completely and accurately you will be entered in to a quarterly drawing to win an Apple iPod Nano 8G. The drawing is open to residents of the 50 United States (and D.C.) and Canada, 21 years or older, who own or are employed by a registered business in the swimming pool industry. Prizes will be awarded quarterly. Quarterly Sweepstakes winner will be selected through a random-computerized drawing of the week after the quarter’s closing dates. (There will be four sweepstakes winners in a calendar year.) Winning entry will be verified and winner will be notified by either mail, phone or email within 30 days of the drawing and may be required to execute a Statement of Eligibility and Release within 30 days of prize notification attempt or prize may be awarded to an alternate winner at DECK-O-SEAL’s discretion.
